You are submitting your information for consideration by MonopolyLoan.com and other PayDay Lending Partners.

Description of Disclosures and Consent. You are or may be entitled to receive certain disclosures if you submit your information through the referenced websites, including those that: (a) notify you about the response or decision made by MonopolyLoan.com or a PayDay Lending Partner in connection with your Inquiry; (b) disclose how we generally collect, use, and disclose nonpublic personal information about you; (c) disclose how, with your consent or at your direction to process your Information or provide the financial services you requested, we and our PayDay Lending Partners may collect, use and share about you and your Inquiry; and (d) any related documents or notices we are permitted or required to give you under applicable law, including those required in connection with your Inquiry and any breach of security related to information we maintain about you and your Inquiry (collectively, the "Disclosures").

To submit your Information through the referenced websites, you must give your initial consent to use and accept certain signatures, records, and Disclosures in electronic form. This Consent applies to the Disclosures and any Information you submit for consideration by MonopolyLoan.com and other PayDay Lending Partners through the referenced websites. This Consent informs you about your rights when receiving these Disclosures electronically.

Withdrawing Consent to Electronic Delivery. In these circumstances, we will continue the financing process in non-electronic form at no charge. If you decide to withdraw your consent for electronic Disclosures, you understand and agree that any prior Disclosures provided or delivered to you in electronic form will remain legally effective, valid, and enforceable.

Paper Copies of Disclosures. If you wish to obtain a paper copy of any electronic Disclosures, you may submit your request to us in writing at 13801 Walsingham Road #A-408 Largo, FL 33774. There is no fee for paper copies of these Disclosures.

Email Address of Record; Procedures to Update Email Address. You agree that the most recent email address we maintain for you is the one we may use to provide electronic Disclosures.
